As a critical member of the marketing and admissions teams, the Hospital Liaison works closely with hospital discharge planners, physicians, palliative team members, social workers, case managers, management, inpatient unit and education staff to generate referrals. Develops and maintains new and current relationships with assigned hospital(s). The Hospital Liaison also provides initial and on-going assessments of patients in facility contracted beds, and manages patients admitted to the hospital for symptom management.

JOB SUMMARY:
The Hospital Liaison is a Licensed Nurse who is responsible primarily for developing and maintaining relationships with area hospital nursing, social work staff and discharge planners. The Hospital Liaison will work to ensure a smooth transition for existing patients and clients of MedSide from the hospital to home.
JOB QUALIFICATIONS:
The Hospital Liaison must possess the following qualifications:
- Graduate of an approved Nursing Program.
- Current RN or LPN licensure, with no sanctions, restrictions, or provisions, in the state of Georgia.
- Previous nursing experience, with an awareness of home health and home care requirements.
- *Preference will be given to applicants with bi-lingual skills
